Safety Alert: Top Items to Collect When an Injury Occurs

After every injury collecting the proper items/documentation necessary helps to ensure that the claim can be filed and handled quickly and properly. If there is missing information on the FROI or missing documentation this can effect how quickly a claim can be handled. Please find below the top 10 items to collect after a workplace injury occurs to help avoid any issues with handling a claim:

  1. Report the accident immediately - regardless of how minor the injury or accident may be, it should be reported at once.
  2. After seeking medical attention, get a statement from the injured employee (if possible).
  3. Gather detailed statement(s) from witness’, preferably written and signed by each individual.
  4. Take pictures of the scene and any equipment involved and save any video that may have captured the accident/injury.
  5. Complete the First Report of Injury and complete it with as much detail about the accident and injury as possible - Be sure to add precise information about the exact location of the injured body part. For example, if the injury occurred to the index finger of the right hand, be sure to list it on the first report as such. This will assist in keeping the claim to the specific part of the body in which the injury occurred.
  6. Preserve any maintenance records for any equipment involved.
  7. Document any potential third-party issues.
  8. Gather materials related to any safety policies or procedures related to the activity in which the accident/injury occurred.
  9. Gather all records of safety meetings or training that the injured employee attended. 
  10. Ensure that a post-accident drug screen is performed as soon as possible but within 32 hours of the accident/injury.
